Sri Lanka High Commission in New Delhi receives applications for new passports from Sri Lankan nationals.If at anytime there is no further space for visas or a passport is more than 10 years old or the passport is lost, an application for a new passport can be submitted to the Sri Lanka High Commission in New Delhi. Application forms for Sri Lankan passports are available at the High Commission or can be downloaded from <www.immigration.gov.lk>.

 Application for a new passport

 Required documents

Application form “K” (download)
Old passport or copy of the old passport
Valid original Indian Resident Permit/ Legitimating card if available
Four (04) color photos 4.5 x 3.5 cm (without white boarder)
Original birth certificate or copy
Original National Identity Card or copy
Original Marriage Certificate if the applicant is married female
Form D-3
Photocopies of all the originals
Self addressed cover with stamp if the new passport is to be sent to the applicant by post
Ind. Rs. 6,355.00 fee

It takes about 08 weeks to issue the new passport.

If applying for a new passport/travel document in case of emergency, in lieu of a lost passport, the original Police report with its English translation and duly completed Form ‘D’ should be submitted.

 Identity Certificate:

Identity Certificate will be issued for Sri Lankans only for return to Sri Lanka in lieu of lost passport. The application should be submitted with the following:

Application form “K” (download)
Form “D” -3
Copy of the old/lost passport if available
Valid original Indian Resident Permit/ Legitimating card if available
Four (04) color photos 4.5 x 3.5 cm (without white border)
Original birth certificate or copy, if available
Original National Identity Card or copy, if available
Any other document to be proofed to be Sri Lankan
Self addressed cover with stamp if the new passport is to be sent to the applicant by post

Renewal of passports

Following are the required documents:

Original Sri Lanka Passport
Ind. Rs. 100.00 renewal fee for one year
Photocopies of all the originals

Change of Name in the passport after marriage

Following are the required documents.

Original Marriage Certificate
Consent letter of Husband
Valid original Indian Resident Permit/ Legitimating Card
Photocopies of all the originals
Ind. Rs. 250.00 as fee

Inclusion/ Deletion of children’s name in a passport

Sri Lanka passport
Original Birth certificate of the child whose name to be included or deleted.
Valid original Indian Resident Permit /Legitimating Card
Letter of consent /request letter signed by both parents
Original National Identity Cards of parents
Photocopies of all the originals
Ind. Rs. 250.00 as fee
